                     AccuBase A1c Test Kit

                           Instructions For Use

                                                                                                                         

Lay out the parts of the kit and become familiar with them. Take your time,

the test should go very easily and quickly for you, once you know the kit

parts and their functions. You do not need to be fasting to take this test.

                           Components of the Kit               

 

Part 1:  Reagent Vial; numbered vial containing the

testing solution. 

 

 

 

Part 2:  Capillary Collection Device; this is the

device that will automatically draw a small blood

sample from your drop of blood once you have

lanced your finger. The open end of  this device is placed directly into your drop

of blood.

 

Part 3: Workstation; used to hold the OPEN

Reagent Vial during sample collection.

  

 

 

 

Part 4: Lancet Device; this is the Orange Single Use

Device that contains a sterile sharp needle used to

lance (puncture) your finger in order to obtain a

MEDIUM size drop of blood (more is better). 

 

 

Part 5: Lab ID Form; Complete the requested information.

Print very carefully, if you wish your lab results sent by

e-mail you MUST print very carefully and clearly, so we

can read the information. Typically lab results are ready

with in 24 hours after arriving in the testing lab. Keep the

YELLOW copy (with number) for your records in case you want to track the results.

 

Part 6: Return Mailer; this is the Postal mailer that will contain the Workstation,

    completed lab form and zip-lock

    bag containing your reagent vial

    with your blood.

 

 

     Part 7: Zip Lock Bag; Used to contain the

   vial during mailing.

 

 

                               

 

 

             Sample Collection Instructions:

     

    Step 1.  Place the Workstation in front of you so you can easily reach it.  Open and

    place the reagent vial in the STAR hole as Shown.  

    Use both hands to perform this procedure. Open it

    low and away from your face.  Now place the OPEN

    vial in the STAR burst hole. This will keep it safe from

    spilling and allow you to perform the necessary next

    steps. 

 

       Step 2.  Decide on a puncture site on one of your larger fingers.

        Remove the clear cap from the Lancet Device. 

        Have the Capillary Device ready to pick up as soon

        as you have punctured your finger.  Place the RED

        pad against your selected puncture site and FIRMLY

        press the device against the finger. Don’t be shy!!  

        Press it. You will hear it click, indicating that it has performed its job.  Gently

        squeeze the finger just below the punctured site.  Keep your hand below the

        level of the heart so the blood will flow down easily.  Concentrate on getting a

        medium size drop of blood.  As this may be the first time you have performed

        such a task, a spare lancet is provided.  

 

 

 Step 4.  Pick up the Capillary Device with your free hand. Position it in the blood

 drop as shown to allow the blood to run into the

 tube. DO NOT SQUEEZE the Bulb in this step.  You

 should position the device in a straight line with the

 tip of the finger so the blood will run horizontal. 

 This allows the blood to quickly draw into the device

 to the 2 BLACK Lines (- -).  This is all the blood that

 is needed. A spare tube is provided just in case you

 have trouble. 

 

  Do not tilt the open end of device upward, you want

 to keep the device containing your blood pointing

 down as your transfer the sample to the reagent vial.

 

 

Step 5.  Position the open end of the capillary tube

in the reagent solutionas shown.  Now you can

SQUEEZE the Bulb.  Squeeze once and release the

pressure, squeeze it again and this time maintain

the pressure as you remove the device from the

reagent solution.  Once the solution has turned

PINK, remove the tube.

 

Step 6.  Now close the Purple Cap and remove it

from the Workstation.  You will now place the

vial in the zip-lock bag and return it along with

the White copy of the lab form to the Workstation. 

 

 

 Step 7.  Return the Workstation

to the Postal Mailer as shown.

The kit is engineered to withstand a

significant amount of stress if the

Workstation is placed in the correct

position for mailing.

 

 

     

 

 

      

       Step 8.  Peel the self adhesive strip and close the Mailer. 

    It has all the postage that  is necessary to mail

    (First Class) back to the laboratory.  DO NOT label

    over the return address, it is designed to always be

    returned to DTI Laboratories.  You must place the

    workstation back into the mailer the way it came when

    you return your sample (as Shown). The mailer is approved for safety by the USPS

    and does not need to be taken to an agent or clerk. It can be mailed in any USPS

    slot or mail box.  You are now ready to mail your sample to the laboratory.
